Common Policy Errors

Identify the right evidence and recovery task for common Policy failures.

Use the action and response together when identifying an error. Keep application programming interface (API) response details with your incident evidence. The same status code can describe different problems. Keep returned details and support references before attempting recovery.

Find the Matching Symptom

A member source is a directory or identity source configured in Policy Integration Manager (PIM). Its connection metadata differs from Policy’s own service login or a human user’s token.

Use this table to match an error symptom or response with its interpretation and next task:

Symptom or ResponseInterpretation and Next Task
401 on an authenticated requestRestore your supported gateway session through identity troubleshooting.
403 on a policy commandCheck command capability and applicable resource grants through authoring and review troubleshooting.
428 on a conditional commandSupply the correct resource validator through token troubleshooting.
400, 409, or 412 after another user changes workCompare current content and validators through token troubleshooting.
409 or 422 with validation findingsSeparate author corrections from operator configuration through validation troubleshooting.
shared-protector-permission-escalation or shared-protector-permission-unverifiableReview shared source permissions with the integration owner. Do not acknowledge unsafe widening as a reviewable exception.
Missing or invalid Protector LocationAsk the PIM integration administrator to check the selected protector and its Internet Protocol version 4 (IPv4) address range, then revalidate.
Unmatched classificationConfirm current classification assignments and discovery with the source owner. Do not claim protection for unmatched data.
Invalid member-source configurationRetain the sanitized field path and support reference. Ask the identity integration owner to correct source configuration.
Empty identity provider (IdP) connection data after installing Policy later than TraverseSave the existing connection details again using the backfill workaround.
Approved policy remains Publishing or EnforcingRead the latest Operations request and follow publication recovery.
Publication reports a role without membersAsk the identity integration owner to verify role membership and synchronization before considering retained-artifact retry.
Missing native ownership identityInspect the policy incident through ownership reconciliation.
503 during ownership inspectionRestore complete inventory access. Do not interpret the response as proof that a native object is missing.
412 during binding enumeration or reconciliationRestart enumeration or inspect again through stale-binding cleanup.
Source changes produce no new compilation versionDistinguish unchanged output, blocked cycles, and deferred work through recompilation troubleshooting.
Display-only imported policy offers no editingUse the supported authoring path for a new governed policy. Do not change ownership records to make imported content editable.
Workload startup, readiness, or Panel route failureFollow installation, identity, and routing troubleshooting.

Configuration errors can require an administrator or a dependency owner. Restricted diagnostic access does not imply permission to change that dependency. Refer to diagnostics for the handoff.

Verifying the Common Errors

Verify the outcome of the failed task, not merely service health.

Use these checks to confirm the task’s result:

  • Reload the draft and check saved content and status after a correction.
  • Check the new approved version in Change History and its delivery request in Operations after approval.
  • Follow the Data or Guardrails verification guide after publication recovery.
  • Check saved mappings after ownership repair or cleanup.
  • Check routing and readiness separately after platform recovery.

Liveness and readiness checks cannot prove policy publication or target protection. Use the relevant playbook’s verification before closing the incident.
